- Your employer’s name and address
- Workers' compensation claim number
- Date of work related injury
- Insurance carrier’s name and address
- Insurance adjuster’s name, phone and fax numbers
PLEASE BRING THE FOLLOWING INFORMATION, WHEN POSSIBLE,
TO YOUR APPOINTMENT:
- All medical records involving your injury, including: initial and subsequent evaluations/consultations by other practitioners, including Independent Medical Examination report(s), operative reports of previous surgeries.
- All other information as requested under WHAT TO BRING WITH YOU
