We understand you may have questions about your care and treatment options. We encourage you to ask those questions. Keeping in constant communication helps us determine whether your are on the right path or may need an adjustment in your treatment plan.

What to expect during your first visit

When choosing Neurosurgical Care, you can feel confident to receive a thorough evaluation of your symptoms and medical history from one of our staff members. Upon arrival you will be escorted to our consultation office, an environment where you will feel comfortable and relaxed.

What we will discuss
  • Your medical history: Be sure to have this information ready when you arrive
  • Your symptoms: The most important information your health care team needs involves your symptoms (such as where you have pain, weakness, numbness, etc.) and what limitations these symptoms place on your everyday activities. Please take the time to write down this information.
  • Your diagnostic tests: Any types of tests, such as MRI, CAT-scan, or X-ray that were performed prior to your consultation will be reviewed. Be sure to bring these with you to your first appointment as they will be critical to determining which treatment option is best for you.
What to bring with you
  • Completed Patient Registration Form(s)
  • Completed Case Registration Form (for work-related injury or motor vehicle accident patients only)
  • All applicable diagnostic studies, including MRI, MRA, CT, Myelogram, Arteriogram, Discogram/Discography, X-rays, EMG and nerve conduction studies. These studies are critical to your examination. You may bring a disc or copies of the films, as the facilities will not release originals.
  • Other medical records relevant to your visit. These may include: physician's notes, physical therapy report, operative report(s) of previous related surgeries, legal documents.
  • Your insurance card, accompanied by a photo ID
  • Insurance referral from your PCP if applicable
  • Your copay if applicable which can be paid by cash, check, or credit card and will be collected at the time of your visit.

After reviewing your medical history and diagnostic tests, you will be presented with a number of treatment options. We will inform you of the risks and benefits of each option and will assist you in making the final decision concerning which option is best for you.

Keep in mind…

You are the only one who can decide which treatment is best for you. It is important that you recognize that you have a responsibility to participate in, and take ownership of, any decisions involving your health care. You will be asked to make the final decision about what is best for you, so ask questions about anything you do not understand.
